Commercial Application For Utility Service

All applications are subject to review and approval. The owner of the business or an individual that has the authority to sign on-behalf of the business owner must complete the application. If the business is a Corporation, LLC, Partnership or other, please provide proof on company letterhead that the individual has the authority to sign on-behalf of the business. The applicant may complete and submit a residential application for utility services at any time, however, applications are processed during regular business hours of Monday through Friday, 8:00 a.m. to 5:00 p.m. Our Customer Service Representatives will gladly answer any questions regarding the application during these hours, please contact us at 435-627-4700.
Please allow at least two to three business days from the date you are requesting services to be connected for the application to be processed. The driver’s license is required for the person signing the application. If the applicant is a Sole Proprietorship or Partnership, the Social Security Number is required for each individual. All of your personal information is kept confidential. We do not share or sell your personal information.
A security deposit is required in order to receive services. A City representative will contact the business to obtain payment of the security deposit. Services will not be rendered until the security deposit is paid.

Consumer Responsibility

By initialing here, you acknowledge that you will be charged a $25.00 per meter fee on your first bill.
Please come into the office if you wish to have the services connected on the same day. The fee to connect services on the same day is $50.00 per meter. Office hours are Monday -- Friday 8:00am -- 5:30pm.

Payment
The applicant agrees to pay monthly for the utility services rendered by the City of St. George. Charges for service will be made at the regular established rates for the class of service applicable to the applicant. It is the consumer’s responsibility to review the monthly bills for accuracy and notify the City of any concerns. A fee will be charged on return payments per the City rate schedule.

Delinquency
Payment for services is due immediately upon billing and shall become delinquent if not paid by the due date reflected on such billing. A late charge of 5% per month of the unpaid balance will be added to delinquent accounts. The applicant agrees to pay reasonable expenses of collection including collection agency fees, attorney's fees, interest fees, and court costs should it become necessary to use such measures to collect the charges. The City shall charge a processing fee when a shut off notice is sent to the applicant per the City rate schedule. The City shall terminate services on delinquent accounts not paid after notice. In order to restore service the applicant must bring current all delinquent charges. In addition, the City shall charge a reconnection fee.

Security Deposit
The applicant is required to pay a deposit in order to receive service. The City will pay interest on the deposit at the rate set by the City Council. It is agreed by the applicant that the deposit is not considered as prepayment of any bill. Unpaid accounts will be considered delinquent notwithstanding the existence of a security deposit. The City may apply the amount of the security deposit (including interest earned) to the applicant's final bill and any balance remaining will be refunded to the applicant.

Reasonable Access
The applicant shall permit the City's authorized representatives to enter onto the applicant 's premises at all reasonable times for purposes connected with rendering, billing, or disconnecting utility services. Services may be terminated if reasonable access is not permitted. It shall be the responsibility of the applicant to check water pipes, fixtures, and electrical outlets at the service address before requesting water and electrical services to be activated. The City shall not be responsible for damage caused to the property when services are activated.

Termination of Service
The applicant agrees to be responsible for the payment of utility charges incurred at these premises until their responsibility is terminated in one of the following ways:
  1. By mutual agreement evidenced in writing and signed by the City and the applicant.
  2. By a two day written notice from the applicant to have services disconnected and the City physically terminating the service.
  3. By the proper assumption of the payment responsibility by a party acceptable to the City and upon completion of an application for service by the other party.

Applicant warrants that Applicant is the Business Owner or is authorized by the Business Owner for this property to apply for services and that all the information provided is true and correct and understands that false or misleading information shall cause the City to deny or cancel service and demand immediate payment of any amounts which are due.
